Short Term Disability Benefits
are provided to protect your income in the event that you are unable to work due to a non-occupational accident or sickness. Weekly benefits are adjudicated and paid by Alicare, Inc. The benefit is available to active employees only. The amount of the weekly benefit is based upon work hours and credits earned during the prior Contract Year. To apply for Short Term Disability payments, complete the "Telephone Claim Easy Reference Card" found on page 30 of the Summary Plan Description and call 866-975-4090, Monday through Friday, 8 AM to 8 PM.
Effective July 1, 2010 the following schedule is in effect for claims made on or after that date:

Maximum Benefit Period:
52 weeks: Non-work related illness or injury
26 weeks: Work-related illness or injury, subject to repayment
